Fix cleanup apply pagination restart

Summary

• Restart mutating cleanup/reconcile continuation commands at offset 0 after writes/removals change the candidate set.
• Keep read-only pagination unchanged.
• Teach abandoned cleanup drain to treat restart-at-zero as intentional continuation.

Fixes #800.
