Fix cleanup revalidation candidate state

Summary

• Split bounded cleanup apply planned candidates from processed candidates.
• Add final_action/final_reason_code plus fresh dirty/unpushed revalidation fields to processed candidate rows.
• Add a regression test for stale planned dirty state being replaced by fresh blocker state.
